The business and its commitments

Identify the entity, locations, contracts, leases and other commitments within the proposed review. Record who supplied each document, which period it covers and whether the relevant professional has examined it. SBA identifies these business materials as matters to investigate with appropriate assistance. Our request list is a starting structure that the actual reviewing team can adapt.

The services being delivered

Compare written descriptions with the service sequence. Ask about the population served, admission criteria, treatment professionals and family participation. SAMHSA informs the credential, treatment-method, family-participation and broader-support questions. Population and admission questions are original editorial prompts for the responsible clinical leaders. Document a claim and its source separately from a conclusion about clinical suitability.

The operating responsibilities

Map the people who receive inquiries, arrange admission, maintain records, coordinate outside care and respond when coverage changes. A policy describes an intended process. Evidence of execution may require a documented example, a current schedule or a conversation with the people doing the work.

Continuity during a change

List the contacts and responsibilities that may change. Ask how people receiving care will know whom to reach and how authorized information will reach the responsible team. HHS provides a primary reference for mental and behavioral-health information sharing; a change in business ownership does not answer every permission question.
